To make sure that all necessary information is put on your certificate of analysis, we ask our customers to fill in an analysis request form or to insert the samples in our extranet database (accessible on this website through the client portal). If you don't use the extranet, you can download a blanc form below and add it to the sample. 

However, we seriously recommend using the extranet database to register your samples. Please contact us to discover all advantages of using extranet.
